GREEN — Pebble Creek is a 150-bed senior care and rehabilitation facility located at 670 Jarvis Road.

The 28-year-old facility is owned by CommuniCare Health Services and is Medicare- and Medicaid-certified, according to its website.

“Our community offers residents a comfortable environment, along with the benefits of various levels of personalized care,” said Tari Northcutt, director of public relations for Pebble Creek. “It is a place to recover from hospitalization.”

Patients come to the facility for short-term and long-term care, according to Northcutt. Once a patient is admitted, staff physicians, nurses, therapists and social services personnel use an interdisciplinary approach to help patients recover, she added.

“We accept all kinds of patients needing therapy, including cardiac, cancer, pulmonary, respiratory and high acuity patients,” said Northcutt. “Some of our therapy services include physical, speech, occupational and respiratory therapy, as well as audiology and wound care.”

Pain management, relocation and stress management and therapies that include music and art also are available.

Northcutt said staff at the facility work hard to get patients back home after a serious injury or illness. 

“We begin the home-going process before a patient is even admitted to Pebble Creek with a visit from our hospital liaison,” she added. “We offer tailored therapy programs for each client, including outpatient therapy, if needed, after discharge.

“We have the most advanced therapy equipment in Northeast Ohio. That equipment includes the ‘Alter-G’ anti-gravity treadmill, which specializes in maintaining a fall-safe treadmill workout for our rehabilitation clients.”

Northcutt said Pebble Creek is the only facility in Ohio that features a Life Trail Fitness Path for its clients. The trail is composed of 10 outdoor “Wellness Stations” that provide fitness, strengthening and balance exercises.

During a patient’s stay at Pebble Creek, a culinary team can modify menus and prepare food to accommodate a client’s personal needs and preferences, according to Northcutt. Clients are offered a choice of meals from the menu for in-room dining.

The facility has private and semiprivate suites, and patients are invited to personalize their space with photographs, plants and other small items.

Pebble Creek has secured outdoor access to ensure residents are safe, according to its website.
